PRIVATE NAVIGATION

Personalized Healthcare Navigation When the Situation Becomes Too Complex to Manage Alone

A serious diagnosis, changing care needs, multiple specialists, insurance barriers, care transitions, or an aging family member can quickly create a healthcare situation that becomes difficult for patients and families to manage on their own.

NAVASSIST provides personalized, one-on-one private navigation to help bring structure, coordination, and follow-through to complicated healthcare journeys.

Working alongside the patient's physicians and healthcare team, a dedicated NAVASSIST navigator helps identify priorities, organize the plan, address barriers, coordinate next steps, and remain engaged as the situation evolves.

MORE THAN CARE COORDINATION

Effective navigation is not simply making phone calls, scheduling appointments, or providing a list of resources.

It requires understanding the whole situation, identifying what matters most to the patient and family, developing an actionable plan, coordinating across organizations, following through on unresolved issues, and determining whether the intended outcome actually occurred.

NAVASSIST has built its private navigation model around those principles.

A STRUCTURED APPROACH TO NAVIGATION

NAVASSIST's navigation methodology is informed by the principles underlying Medicare's Principal Illness Navigation (PIN) and Community Health Integration (CHI) services—federal frameworks that recognize the importance of coordinated, person-centered support for patients navigating serious illness and barriers to care.

Our private navigation process includes:

ASSESS

Understand the patient's healthcare situation, current providers, immediate concerns, support system, barriers, priorities, and desired outcomes.

ESTABLISH PATIENT-DRIVEN GOALS

Identify what the patient and family are trying to accomplish and establish clear navigation priorities.

DEVELOP AN ACTION PLAN

Translate those priorities into specific next steps, responsibilities, resources, and follow-up needs.

COORDINATE

Help connect providers, appointments, testing, referrals, insurance processes, community resources, caregivers, and other parts of the healthcare journey.

FOLLOW THROUGH

Track outstanding issues and determine whether planned actions actually occurred.

REASSESS

Determine whether the problem has been resolved, whether circumstances have changed, and whether additional navigation, escalation, or a new approach is needed.

DOCUMENT

Maintain an organized record of navigation activity, communication, barriers, actions taken, outcomes, and next steps.

WHEN PRIVATE NAVIGATION MAY HELP

Private navigation may be especially valuable when:

  • A new or serious diagnosis has created an overwhelming number of decisions and next steps
  • Multiple physicians, specialists, facilities, or healthcare organizations are involved
  • Referrals, testing, treatment, or follow-up are being delayed
  • Insurance authorization or coverage issues are interfering with care
  • A patient is transitioning between the hospital, rehabilitation, assisted living, memory care, or home
  • Adult children or other caregivers are trying to coordinate care from a distance
  • The family is unsure who is responsible for keeping the healthcare plan moving
  • Caregiver responsibilities have become difficult to manage
  • A healthcare situation has become too complicated for the patient or family to navigate alone

WHAT YOUR NAVIGATOR CAN HELP WITH

Depending upon the needs of the patient, navigation may include:

  • Healthcare system navigation
  • Care-plan organization and follow-through
  • Physician and specialist coordination
  • Appointment, referral, testing, and treatment coordination
  • Insurance and authorization follow-up
  • Communication across healthcare organizations
  • Identification and resolution of barriers to care
  • Care-transition support
  • Organization of relevant healthcare information
  • Community resource identification and connection
  • Caregiver coordination and support
  • Preparation for important healthcare conversations
  • Ongoing monitoring of unresolved healthcare needs

A RESOURCE FOR FAMILIES — AND THE PROFESSIONALS WHO ADVISE THEM

NAVASSIST also works with professionals whose clients encounter healthcare situations that extend beyond the professional's primary area of responsibility.

Elder-law attorneys, life-care planners, fiduciaries, financial professionals, senior-care professionals, and other trusted advisors may encounter clients whose legal, financial, housing, or long-term-care planning is being complicated by an unresolved healthcare issue.

NAVASSIST can step into the healthcare-navigation lane—working directly with the patient and family while allowing the referring professional to remain focused on their own area of expertise.

Our role is complementary.

We do not replace physicians, nurses, attorneys, case managers, social workers, or other professionals involved in the client's care. We help connect the moving pieces and provide focused follow-through when healthcare coordination becomes complex.

ONE CONSISTENT POINT OF CONTACT

Complex healthcare journeys frequently involve many different organizations, each responsible for only one part of the patient's care.

NAVASSIST provides something that can otherwise be difficult to find:

One consistent person helping the patient and family understand what needs to happen next—and helping keep those next steps moving forward.

PROFESSIONAL, NON-CLINICAL NAVIGATION

NAVASSIST works alongside the patient's existing physicians and healthcare team.

Our services are non-clinical and do not include medical diagnosis, treatment, or medical advice. Clinical decisions remain between the patient and their licensed healthcare professionals.

NOT SURE WHETHER NAVIGATION IS NEEDED?

Sometimes the first challenge is simply determining where to begin.

NAVASSIST offers a complimentary Navigation Consultation to discuss the situation, identify the primary navigation concerns, and determine whether private navigation may be appropriate.
