At NAVASSIST, we believe patients with complex and multiple diagnoses benefit from supportive services between clinic visits and hospitalizations. Providing patients with a reliable, knowledgeable and friendly point of contact for non-clinical coordination activities can lighten the emotional and physical toll of a difficult diagnosis.

Our team of non-clinical professional navigators has years of experience in the healthcare industry. They understand the nuances of Medicare, the importance of timely testing and follow-ups, and the stresses of scheduling and prescription refilling. They are here to facilitate all of your treatment plan coordination activities with skill and compassion.

Our work is guided by empathy, respect, and a deep commitment to patient dignity. Every interaction is grounded in compassion and understanding. We do not judge. We support without exception. And we remain focused on the single most important priority — you.

Patients often leave visits feeling overwhelmed about what happens next. They need referrals, follow-ups, tests, scheduling assistance, help with insurance concerns and overall emotional support. A fragmented healthcare system makes things disjointed and confusing.
NAVASSIST supports patients with all of their coordination activities, decreasing their stress levels and helping them stay on track with their care plan.

NAVASSIST provides remote, non-clinical patient navigation services to support Medicare patients in following through on complex treatment plans developed by their physicians and care teams, managing coordination activities between clinic visits and hospitalizations.

At NAVASSIST, our mission is to provide support for coordination activities between clinic visits, when things can fall through the cracks and non-clinical navigation can be invaluable. atients receive highly personal and focused support.

NAVASSIST navigators bring strong healthcare experience and a practical understanding of how complex care systems affect patients. Many have years of clinical experience, giving them a unique understanding of the challenges faced by patients with complex diagnoses.
Our role is strictly non-clinical, while our clinical perspective and experience is grounded in real-world care delivery. We provide structured guidance, consistent follow-through, and dependable support so patients stay on track and clinics can stay focused on care delivery.

Care plan success depends on what happens after patients leave the clinic. NAVASSIST provides structured, non-clinical navigation that supports follow-through on referrals, testing, appointments, and next steps. We reinforce instructions, close care gaps, and keep Medicare patients aligned with their plan of care.

Not exactly. Case management is often insurance-driven and may be episodic and only available during hospitalizations or specific events. Patient navigation is ongoing, patient-centered support focused on helping you carry out your physician’s plan and addressing real-life barriers between visits, when your support options are slim.
Patient navigation is especially helpful for:
Navigators do not diagnose, treat, prescribe, or make medical decisions.
They support the care plan created by your physician and help remove practical barriers that may prevent you from following through. Our navigators are not clinicians and are trained to refer any clinical concerns immediately back to the clinical practice .
Patient navigation is non-clinical support that helps patients follow through on their physician’s care plan. Navigators assist with coordination tasks that occur between medical visits — scheduling, reminders, resource connections, and follow-up — so care stays on track.
