    How long does patient navigation last?


    Services are typically provided on a monthly basis and can continue as long as your physician determines support is medically appropriate and beneficial.


    Is my information secure?


    Yes. NAVASSIST follows HIPAA privacy and security standards to protect your information. We are fully committed to your privacy and protecting your health records.


    How often will I hear from my Navigator?


    Support is personalized based on your needs. Some patients benefit from weekly check-ins; others require less frequent contact. We tailor support to your care plan.


    Are services provided in person?


    No. NAVASSIST operates remotely using secure communication methods (phone, video, secure workflows). This allows us to serve patients consistently, efficiently, and across geographic regions while staying connected to your care team.


    What does NAVASSIST Not Do?


    We do not:

    Does Medicare pay NAVASSIST directly?


    When services qualify under Medicare guidelines, reimbursement is made directly to NAVASSIST LLC. Patients are not responsible for managing Medicare billing.


        

    Do I need a referral from my doctor?


    Yes. Medicare-covered navigation services require a physician or qualified healthcare professional to initiate or approve services as part of your care plan. At NAVASSIST, we respond to physician referrals within 24 hours. Additionally, we reach out to clinical practices on behalf of patients that request our services directly through other channels.


        

    Is NAVASSIST patient navigation covered by Medicare?


    Yes. Medicare recognizes Primary Illness Navigation (PIN) services when ordered by a physician and provided under appropriate supervision and documentation standards. NAVASSIST is becoming a licensed provider of PIN services and able to bill Medicare for reimbursement directly.

    Is this the same as case management?


    Not exactly. Case management is often insurance-driven and may be episodic and only available during hospitalizations or specific events. Patient navigation is ongoing, patient-centered support focused on helping you carry out your physician’s plan and addressing real-life barriers between visits, when your support options are slim.


        

    Who can benefit from NAVASSIST patient navigation?


    Patient navigation is especially helpful for:


    • Patients managing chronic conditions or several acute & chronic conditions

    How is NAVASSIST patient navigation different from nursing or medical care?


    Navigators do not diagnose, treat, prescribe, or make medical decisions.
    They support the care plan created by your physician and help remove practical barriers that may prevent you from following through. Our navigators are not clinicians and are trained to refer any clinical concerns immediately back to the clinical practice .

    Patient navigation is non-clinical support that helps patients follow through on their physician’s care plan. Navigators assist with coordination tasks that occur between medical visits — scheduling, reminders, resource connections, and follow-up — so care stays on track.
